Medicaid – Case Management for Children and Pregnant Women

Are you Medicaid eligible, pregnant, and have a high risk condition? Do you need help accessing medical, social, educational, or other services related to this condition? If you answered yes, this may be the service for you!

Case Management Eligibility Criteria

In order to be eligible for case management services, you must be:

  • Medicaid eligible
  • Be pregnant and have a high risk condition OR be a child 0-20 years old with a health condition and a health risk
  • Need help accessing medical, social, educational or other services (including non medical) related to their health condition/health risk or high-risk condition
  • Want help accessing services related to their health condition/health risk or high-risk condition
